Mandatory Medical Form Procedure for Fall 2017
All players must fill out the medical athletic forms, prior to tryouts, forms can be found on the Ridgewood Athletics Page.
Medical forms to be returned to nurse's office:
1) Pre-Participation Physical Evaluation (PPE) -- please note, there are four parts to this form and it is valid for one year from the date of the physical examination. If you have submitted this form for a previous high school sport and it is still valid, you do not have to hand in this form, but need to fill out the Health History Update Questionnaire.
2) Health History Update Questionnaire--in lieu of the PPE if the PPE has been submitted for a previous sport and the physical examination date is within a one year time period.
*Please note that it is not acceptable to mail in the medical forms (PPE or Health History Update Questionnaire forms) or leave them in the nurse's mailbox at the high school. Medical forms that are mailed in or dropped off at the high school will not be accepted and will be returned! All medical athletic forms must be submitted IN PERSON to the school nurse prior to the end of the 2017-2018 school year or during designated collection days this summer.
The designated summer collection dates are as follows:
- Prior to June 20 during the school day
- Wednesday, July 19 and Thursday, July 20 from 9 am - 12 pm
- Monday, July 24 and Tuesday, July 25 from 9 am - 12 pm
- Monday, August 7 and Tuesday, August 8 from 8 am - 12 pm
- Monday, August 14 from 9 am - 12 pm
